Insurance

Insurance information for Medical Conditions

Our practice accepts the following insurance plans for the evaluation and treatment of medical conditions.

Aetna, Cigna, Blue Cross Blue Shield, Medicare, Multiplan, PCHS, Oscar, Oxford, United Healthcare.

Please email info@entierederm.com with the front and back image of your insurance card and which board certified dermatologist you are seeking to see for insurance verification.

Payment for any deductibles and co-payments is due at the time of service. Health insurance plans do not cover procedures that are not medically necessary, which includes all cosmetic procedures.

Securing referrals is the responsibility of the patient. If a claim is denied due to an invalid, incorrect, or missing referral, the patient is responsible for the cost of the visit.

Insurance plans vary on their coverage and may provide only partial reimbursement of our fees. It is important to understand what your insurance plan is. If we do not participate with your insurance plan but your plan has "out of network" benefits, we are happy to provide the necessary documentation for submitting an "out of network" claim. If we do not participate with your plan, you will responsible for full payment at the time of service,


What to Bring

For all patients at each visit, please bring your photo ID, insurance card, a valid credit card, and review our most updated Covid-19 practice policy


For New Patients - Cosmetic Consultation:

If you are interested in receiving treatment the same day as your one-on-one consultation, please let us know when scheduling the appointment. We will make every effort to accommodate your busy schedule.

The fee for new patient cosmetic consultation is $400 with Dr. Melissa Levin and $350 for Dr. Marie Leger, which is fully credited towards your cosmetic treatment performed within 30 days of the consultation. The consultation fee is non-refundable and non-transferable.


Before Your Visit

With any medical grade cosmetic treatment, we want to ensure you have the best experience. This is achieved by preparing with simple tips to prepare for the procedure. Please contact the office at info@entierederm.com for care instructions specific to your treatment.

With any light or laser treatment, avoid a suntan or sunburn.

Drink adequate amounts of water prior to the appointment.

Avoid alcohol three days before your treatment.

If medically approved, avoid Aspirin, Ibuprofen, vitamin E, and fish oil 7 days prior to treatment.

Don't plan a work-out class or massage immediately after the treatment.


Checking In

After booking an appointment with us, patients can login our secured web portal to input or update your contact information and health history as well as sign all required forms. Access to these secure web portals can be found in the Patients page.

For new appointments, please arrive 15 minutes prior to your scheduled appointment time to fill out any intake forms or update your information.

For all our existing patients, we ask you arrive 10 minutes early to register and enjoy our space.


Cancellation,
Missed Appointments
and Late Arrivals Policy

 

Return Policy

Entière Dermatology is committed providing all our patients with the best care. No shows, late arrivals, and last minute cancellations prevent another patient from being cared for. Entière Dermatology reserves the right to reschedule late arrivals of more than 15 minutes.

We require three-business days (72-hours) cancellation notice prior to cancelling or rescheduling an appointment. If your appointment is scheduled on Monday, please inform us the previous Wednesday morning.

Rescheduling, cancelling, or no-shows within 72 hours of the scheduled time will incur the following fees: $100 for medical appointments and $250 for cosmetic and hair restoration appointments. The cancellation fee is non-refundable.

 

Items purchased at Entière Dermatology (the “Practice”) may be eligible to be returned. To be eligible, the items must be (1) returned within 14 days of receipt, (2) must be returned unopened/unused and (3) in their original packaging. Any items that are determined to be damaged, open or used, at the Practice’s sole discretion, will not be eligible for a return. 

If the item is accepted by the Practice, the refunded amount will be credited to your account and can be used towards another product or towards any outstanding or future payments owed to Entière Dermatology.
